Services Available
Hospice provides terminally ill patients and their families physical, psychosocial, and spiritual support. Care may be provided in the home, assisted living, nursing home, retirement center, hospital, or in one of our hospice houses. Services are available for patients with a life-limiting illness including heart disease, renal disease, neurological disease, cancer, lung disease, Alzheimer’s, dementia, and other conditions. Support is provided to the patient through 4 levels of care:
| Routine Care |
Provided in the patient's home, retirement home, assisted living, or nursing facility. |
| Respite Care |
Patients may stay in one of our hospice houses or contracted facilities for up to five days. |
| Crisis Care |
Provided wherever the patient calls home to control acute pain and symptoms. |
| Inpatient Care |
In the comfort of our hospice house or other inpatient setting, we provide transitional care and acute pain & symptom management. |
