Community Healthcare of Texas Asks – What’s Your Plan?

FORT WORTH, TX, April 13, 2022 – In celebration of National Healthcare Decisions Day on April 16, Community Healthcare of Texas, the state’s largest not-for-profit hospice provider, has created What’s Your Plan, an advance care planning toolkit now available on the organization’s website at www.chot.org/whats-your-plan.

“We have witnessed first-hand the challenges families and caregivers experience when they aren’t aware of their loved one’s wishes when facing a serious or life-limiting illness,” said Viki Jingle, CEO for Community Healthcare of Texas. “That’s why our What’s Your Plan toolkit was so important for us to add to our website. We wanted to make free resources easily accessible to every family so they can make a plan, put that plan in writing, and give their loved ones peace of mind.”

In addition to advance care directives, the What’s Your Plan toolkit includes Conversation Starter Cards to help families walk through what can often be difficult discussions. Also part of the toolkit, Community Healthcare of Texas has created The Gift, a downloadable booklet to outline personal information and wishes. It is designed to provide basic information to family members about assets, liabilities, and personal desires when they cannot convey that information themselves. It also includes a detailed checklist for survivors after the death of a loved one.
