New Resource Combats Heart Failure in North Central Texas
Community Healthcare of Texas, in collaboration with the American Heart Association (AHA) and the National Partnership for Healthcare and Hospice Innovation (NPHI), has launched a new Advanced Cardiac Care Program featuring patient and family educational resources.
Each year, more Americans die from heart disease than any other condition, including cancer. Heart disease is also a leading cause of hospitalization: patients with congestive heart failure alone account for more than one million inpatient admissions annually. Unfortunately, evidence indicates that congestive heart failure patients often do not receive care that aligns with evidence-based best practices and that clinicians and patients are not aware of critical information and resources available through hospice and palliative care.
“It is an unnecessary tragedy that only five percent of heart failure patients who are discharged from the hospital ever utilize in-home hospice or palliative care services,” said Viki Jingle, Chief Executive Officer for Community Healthcare of Texas. “Through our collaboration with AHA and NPHI, the Advanced Cardiac Care Program provides the clinical protocols and accessible patient/family resources designed to advance care and improve quality of life for those facing heart disease.”
The Advanced Cardiac Care Patient and Caregiver Handbook is a comprehensive resource available at no cost for patients and their caregivers and includes valuable information for symptom management, common medications, dietary recommendations to help manage advanced cardiac disease, self-care tips, and more.
