Community Healthcare of Texas provides expert medical care for patients and emotional and spiritual support for the entire family. Hospice care is for individuals who have been diagnosed with a life-limiting illness with a prognosis of six months or less. Our mission is to provide compassionate end-of-life care services wherever you call home. Our hospice team will develop a plan to meet the unique needs of each patient and their family. Medications, medical equipment, and medical supplies are provided to manage the patient’s illness. Our goal is to provide care and support while keeping patients comfortable wherever they call home and surrounded by loved ones.
Patients and family members may call and get information about hospice services at any time without obligation. Anyone can make a referral to hospice. We will work with your physician to evaluate hospice eligibility. Equipment to enhance comforts, such as hospital beds or oxygen, is provided. Acute pain or symptoms are identified, and a plan for management and prevention is developed.
Levels of Care
Community Healthcare of Texas offers four Levels of Care for hospice patients:
Routine Care – Routine home care is provided per an individualized plan of care, wherever the patient calls home. Supplies, durable medical equipment, and medications related to the hospice diagnosis are included.
Respite Care – Respite is short-term inpatient care provided to a patient that allows family members to take a vacation or attend to other family responsibilities. Patients can receive care at one of our many contracted facilities.
Continuous Care – A full-time care team provides this care for relief and control of acute pain and symptom crisis wherever the patient calls home.
Inpatient Care – This care is transitional care for acute pain and symptom management and is provided in an inpatient setting.
Community Healthcare of Texas believes that preparing the patient and family for the changes that occur at the end of life is important. Your hospice team will provide education and support as needed for:
- Preparation for physical changes that occur
- Tips and advice for comfort care
- How to cope spiritually and emotionally while caring for your loved ones

Our team of Nurses, Chaplains, Social Workers, and Physicians are always available to respond to your needs. Depending on where each person is on their journey, the hospice team helps with:
