Every person experiences the end of life differently. A veteran’s military service can add a distinct dimension to the level of physical, emotional, and spiritual support they need. Deep-rooted memories of combat may manifest in different ways and are often recounted as veterans face their own mortality.

Community Healthcare of Texas is honored to offer a Veterans Program to those who have served in our military. Community Healthcare of Texas addresses the unique concerns of veterans and strives to bring them peace, dignity and comfort wherever they call home. Our team of professionals is specially trained to guide you through the life enhancing processes to address individual needs, thus making the Veteran’s Program a natural extension of our mission.

Hospice services are fully covered through the Veterans Affairs (VA) benefits, Medicare, Medicaid and private insurance companies.

Apoyo

Physical comfort and disease management are critical to patient care. Patients and families may also need emotional and spiritual support. Our team of hospice professionals will guide you to address priorities, wishes and needs.

Resolutions

For veterans, deep rooted memories of combat may manifest in different ways and are often recounted near the end of life. During the “final battle,” the team at Community Healthcare of Texas will work to identify and respond to these issues.

Confort

Medications, medical equipment and medical supplies are provided for comfort and convenience while managing the patient’s illness. The hospice team aims to prevent breakthrough pain and prevent uncomfortable trips to the Emergency Room.

Immediate

Acute pain or symptoms are identified and a plan for management and prevention is developed. Equipment to enhance comfort such as a hospital bed or oxygen is provided.

Long Term Pan

A fluid, comprehensive plan incorporates patient goals, history, current symptoms, disease progression, emotional and spiritual needs.

Emergency Symptom Management

• Physician house calls as indicated
• Home “Emergency Kits”
• Support available 24/7
• Continuous Care or Inpatient Care for relief and control of critical symptoms

Community Healthcare of Texas is proud Level 4 partners of We Honor Veterans, a pioneering campaign of the National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs to improve the care veterans receive from hospice and palliative care providers.

Community Healthcare of Texas also now offers a resource guide to assist patients and families in identifying, understanding, and acquiring appropriate veteran benefits through the Veterans Administration and each State’s Department of Veterans Services.
