What is Palliative Care

Community Healthcare of Texas’ Palliative Care Services is an in-home program offering Nurse Practitioner and Physician Assistant visits for patients needing advanced illness support. Through this specialized program, we work closely with your physicians to ensure that your needs are addressed, allowing you to stay safely at home while managing the symptoms, pain, and stress of a serious illness.

The Process

Navigating a serious or terminal illness can be overwhelming and challenging for many patients and families. A referral to our Palliative Care Services program can be made by anyone, including you, a loved one, or a physician. Once contacted, our team will reach out within 3 business days to schedule your first in-home visit with our Nurse Practitioner or Physician Assistant. After discussing your goals, a customized plan will be created to address your individual healthcare needs.

Paying for Palliative Care

Medicare and most private insurance plans cover the cost of in-home visits by a licensed practitioner. The initial visit with our palliative care team is at no cost to you. We will develop a care plan for you during your initial visit and discuss any potential out-of-pocket expenses.
